Our highly trained hospice care providers are here to support and care for you in the last phases of life-limiting illness. - Registered nurse services
- Certified nursing assistant visits
- Care coordinated by your own physician
- Counseling and emotional support
- Medications and medical supplies related to the life-limiting condition
- Inpatient care for periods of crisis
- Respite care to relieve your care giver
- Routine home care delivered to your residence
- Physical, speech and occupational therapies
- Dietary counseling
- Volunteer services
- 24 hours, 7 days a week on-call service
Criteria for Presbyterian Hospice Care
- Diagnosis of a life-limiting illness with a life expectancy of less than six months;
- Knowledge of the diagnosis by you and your family;
- Approval by your attending physician for admission to hospice care; and
- Residency within the service area, which includes the following counties: Brooks, Colquitt, Cook, Lowndes and Thomas.
Who Pays for Hospice Care?
- Medicare
- Medicaid
- Private insurance
- Private payment
Services are provided to all persons who meet our admissions criteria, regardless of race, creed, color, religion, sex, national origin, physical disability, financial status or ability to pay.
